Preservación de la fertilidad en artritis reumatoide: ¿principio precautorio o derecho de información del paciente?
La artritis reumatoide (AR) es una enfermedad que afecta predominantemente las articulaciones de las personas en etapa productiva. Se presenta con mayor frecuencia en mujeres que en hombres, y entre sus signos y síntomas principales se encuentran el dolor y la inflamación. Para su tratamiento se emplean diversos medicamentos, algunos de los cuales se han asociado con efectos adversos sobre la fertilidad. Sin embargo, en varios de ellos la evidencia científica es poco clara, lo que hace necesario que los médicos evalúen el riesgo-beneficio de su indicación con el fin de garantizar el derecho a la protección de la salud, particularmente en lo relativo a los derechos sexuales y reproductivos de las personas. Es importante que dicha evaluación se realice sin prejuicios y sin asumir que los pacientes no pueden o no desean lograr un embarazo, considerando que actualmente existen diversos procedimientos de preservación de la fertilidad (PF) que pueden hacerlo posible. Para abordar esta situación se llevó a cabo una investigación teórico-documental con el propósito de determinar si el personal médico debe informar a los pacientes con AR bajo tratamiento farmacológico sobre la PF desde un enfoque basado en el principio precautorio (PP) —el cual establece que, ante la incertidumbre científica, deben tomarse medidas que eviten o reduzcan un daño potencialmente plausible— o si dicho enfoque debe orientarse desde la perspectiva de los derechos de los pacientes. A partir de lo anterior, se concluyó que el abordaje es pertinente desde ambas perspectivas, ya que son complementarias.
